O: Weighed and assessed body score. R wing drooping slightly in hand and in cage. Able to move R wing. R leg feels weak, but no shifting of bone when palpating. Callous felt on R TBT. Administered oral meds. Collected 61 g of scrap and 3 casts (1 large, 2 small).

A: BAR. R leg a little uncoordinated and feels weak when holding it, but otherwise seems to be healing well. Slight R wing droop remains. Large amount of scrap likely due to E3D catch. Weight down and BCS still at 2.5.

O: R TBT fracture feels stable on palpation, can feel bony callus. R shoulder is very displaced but also feels stable. Took x-rays: bony callus visible at R proximal TBT fracture site. Proximal fragment slightly displaced. Removed pins and took more x-rays: without pins in the way can see better that while there is bony callus and the leg feels stable, there are some areas that need to be healed over more - should continue to heal without pins but need to handle with caution. Drew blood from the L medial metatarsal vein.

A: Patient seems to hold R wing better in cage compared to in hand. R shoulder feels loose on palpation and patient did not really try to pull wing back. Standing fairly well with R leg and pins are doing well. As per AD, TMS appears to have helped with plasmodium load. Will continue TMS until the 2 week mark and recheck blood in a week.
Weight change: +21 g (3 %)

P: Recheck blood in a week. End TMS at 2 week mark.

O: Assessed bird on ground: mild limp on R leg, intermittent knuckling with R foot but seems to ambulate better than expected. Able to flap both wings well but has a slight R wing droop. R TBT fracture feels stable on palpation, R shoulder feels like it has a very slight flex to it when doing full PROM but no crepitus felt. Took x-rays; TBT fracture healing well, shoulder fractures healing but very displaced, possible spinal abnormality noted. Drew blood from the R medial metatarsal vein.

O: See surgery report for details. Partial fracture of the proximal R TBT became a full fracture from the bird struggling in the sling, knee seems intact but felt lax due to smaller TBT fractures close to the joint. Placed IM pin and ESF tie in to stabilize the fractured R TBT.

A: Guarded prognosis due to proximity to joint.

P: Remove from sling. Monitor use of R leg and foot, monitor sutures on medial aspect of R TBT, monitor pin sites.

Surgery report
Procedure - Fracture repair - tibiotarsus
Performed by - AD
Surgery time - 40 minutes

Placed 20g IO catheter in the R dorsal ulna.  Pre-medicated with
torb IM and low dose atropine IM, anesthestized via isoflurane
facemask, then intubated with 3.0 uncuffed ET tube.  Took intraop
x-rays - proximal R TBT fracture, bones in much better alignment
then on intake, proximal fragment looks short and slightly compounded.
Knee joint looks okay.  Plucked feathers on the R TBT and scrubbed
limb for surgery.  Incised over the medial aspect of the leg
at the fracture site, dissecte down to the fracture site and
found the bone ends.  Placed a 2.0 IM pin in the proximal fragment
and out the bent stifle, cut the distal end of the pin and drove
it across the fracture site to seat at the distal end of the
TBT.  Flushed wound site and then closed muscle and skin in separate
layers.  Placed cross pins in the distal and proximal fragments,
took more x-rays to confirm placement.  Proximal cross pin is
a little more proximal/closer to the joint than is ideal, but
try to adjust/remove could risk further damage to the small proximal
fragment, so left as is.  Bent cross pins medially and IM pin
laterally and secured everything together with epoxy.  Pulled
IO catheter and replaced figure-8 and body wrap on R wing.  Bird
recovered slowly but uneventfully from anesthesia.

Abrasion over uropygial gland looks mild, did not clean. Bird acting very visual, only a very small amount of debris in vitreous OU, more in OS than OD. Drew blood from the L medial metatarsal vein, blood pressure seems low.

O: No fecal matter to collect. R leg wrap has slipped distally slightly. Removed and replaced with a sam-splint. L leg too edematous from unabsorbed fluids to collect blood. Tried to get blood from L wing but was unsuccessful. Noted matted feathers on uropygial gland. Gently cleaned with dilute chlorhex and removed some stuck feathers/plant debris with forceps. Applied a thin layer of TAO. Gave meds OU and PO. Placed back in sling in kennel.

A: Barred owl admitted after unknown cause of trauma, likely was hit by a vehicle at some point based on injuries. Has minor ocular trauma, fractured right shoulder should be able to heal with a wrap and cage rest. More concerning is the right leg fracture with possible knee luxation. TBT fx seems to have started to callus already, fibula fracture still very mobile. After discussion with vet willing to try a surgical procedure to stabilize the stifle joint but still has a very guarded prognosis.

P: Rx Meloxicam, Gabapentin, tramadol after 24 hours, fluids, Clavamox, baytril, pentoxifylline,calcium slurry and keto OU. Collect blood and fecal tomorrow. Vet check with surgery on Thursday. Monitor R figure 8 body band - change in 3 days. Monitor R leg wrap - change PRN, monitor for use of right foot and swelling.
